Peer Partner Program is a partnership between HWHA and Mental Health American of Northern California and funded by the Sacramento County Department of Behavioral Health Services. The Peer Partners work as part of the Multi-Disciplinary Team (MDT) at the Aftercare to ensure adults with mental condition can live a quality life. Consumers are referred from the Aftercare Program.

• Eliminate discrimination and help reduce stigma for those seeking self-help services because of mental illnesses.
